What is Help Me Grow Tennessee?

Help Me Grow (HMG) is a nationally recognized, evidence-based model that helps children get the support they need early in life. Instead of creating new programs, HMG strengthens and connects the resources communities already have — health care, early learning, and family support services — into one coordinated system.

How it works:

  • A Central Access Point makes it simple for families and providers to find help.

  • Family & Community Outreach builds awareness of healthy child development.

  • Health Care Provider Outreach ensures concerns are identified and referrals happen early.

  • Data & Feedback are used to continuously improve services.
